  • Duties:

    • Monitor patient’s condition and assess their needs to provide the best possible care and advice

    • Observe and interpret patient’s symptoms and communicate them to physicians

    • Collaborate with physicians and other nurses to devise individualized care plans for patients

    • Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patients’ charts

    • Adjust and administer patient’s medication and provide treatments according to physician’s orders

    Qualifications:

    • RN License is required

    • Prior experience preferred but new grads are welcomed!

  • Duties:

    • Management level nursing position that maintains direct contact with patients and integration with patient care

    • Responsible for coordinating, training, scheduling, and team development among the nursing staff

    • Manages the continuing education of the nursing staff

    • Maintains an ongoing quality assurance program

    • Assists in planning the nursing services portion of resident discharge plans

    • Leads others through actions consistent with and serve our culture

    • Participates in facility surveys by government agencies

    Qualifications:

    • Current RN license

    • Experience in a skilled nursing or long-term care facility

    • 1 year of Director of Nursing (DON)/ Management experience

    • Service-oriented (positive, enthusiastic, empathetic and respectful demeanor)

    • Able to handle stressful situations in a calm and professional manner

    • Capable of organizing tasks and managing multiple priorities

  • Duties:

    • Utilizes evidence-based practice to support clinical interventions

    • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of a Physical Therapist

    • Administers various procedures as part of the rehabilitation plan including but not limited to: manual techniques; ambulation; therapeutic exercises; modalities; use of supportive and assistive devices; etc. Under the supervision and direction of a Physical Therapist

    • Assesses patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements

    • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders

    • Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements

    • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy

    Qualifications:

    • Licensed Physical Therapist

    • This position is open to PTs at all experience levels. New Grads Welcome!

  • Duties:

    • Evaluate patients within forty-eight (48) hours of physician referral

    • Develop effective treatment plans and obtains approval for services from referring physician

    • Treat patients according to the physician's treatment plan

    • Supervise Occupational Therapy Assistants and Aides in direct patient care and patient related activities and adhere to facility Supervision Policy and Procedure Matrix

    • Communicate with supervisor and other health team members regarding patient progress, problems and plans

    • Participate in Patient Care Conference, Utilization Review Meeting and Rehabilitation Conferences

    • Participate in in service training program for other staff in the facility

    • Record evaluations, daily treatment notes as necessary, weekly progress notes, HFCA therapy cert/POC forms per facility policy and procedure

    • Record treatment charges daily per facility policy and procedures

    • Instruct patient's families or nursing staff in maintenance programs

    • Participate in discharge planning

    • Comply with Occupational Therapy Practice Act and the facility Policy and Procedures

    Experience/Qualifications:

    • Licensed Occupational Therapist

    • This position is open to OTs at all experience levels

    • Exhibits a sincere desire to help patients and residents in their rehabilitation process

    • Exhibits knowledge and understanding of therapy practice

  • Duties:

    • Effectively screens/evaluates patients with communication, cognitive or swallowing disorders and develops appropriate plan of care following all regulatory and clinical practice standards

    • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with communication, cognitive and/or swallowing disorders per regulatory and clinical practice requirements

    • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes

    • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the speech disorders

    • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements

    • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy

    Qualifications:

    • A Licensed Speech-Language Pathologist

TN Visas

TN Visas are temporary (up to three years) non-immigrant work visas that permit Canadian and Mexican citizens who work on specific professional occupations to work in the United States.

The occupations of Registered Nurse, Occupational Therapist, and Physical Therapist are eligible.

Immigrant Visas - Permanent Residency

An Employment-Based Third Preference (“EB-3”) Immigrant Visa grants a skilled foreign worker the right to live and work in the United States. An employer can “sponsor” a skilled foreign worker and file an EB-3 I-140 Petition on the worker’s behalf. Immigrant visa numbers are limited each year so the process can take a year or more depending on visa numbers and the country of origin.

H1-b Visas

H1-B Visas are temporary (up to six years) non-immigrant work visas that permit foreign workers in professional occupations to work in the United States. Unfortunately, Registered Nurses are not eligible for H1-B visas.

Occupational Therapists, Physical Therapists and Speech Language Pathologists are eligible.
